What happens when you call.

Six steps, from the first phone call to the year afterwards. No surprises, and nothing you have to work out on your own.

1

Day one

The call

A real person answers, any hour of the day or night. We listen, we answer your questions, and if you are ready, we schedule an admission visit, often the same day.

2

Often the same day

The first visit

A nurse comes to wherever home is, looks at what is needed for comfort, explains everything in plain language, and handles the paperwork sitting beside you. Medications and equipment are ordered before the visit ends.

3

The first week

Deliveries arrive and you meet your team

Supplies and equipment come to the door. You meet your nurse, your aide, your social worker, and your chaplain, and together we build a plan around what matters to your family.

4

Ongoing

The rhythm of care

Scheduled visits, plus a nurse on the phone 24 hours a day in between. Care adjusts as needs change, including short inpatient stays for complex symptoms, or respite so a family caregiver can rest.

5

Near the end

We prepare you, and we stay close

We tell you what you are likely to see, so that none of it takes you by surprise. Support increases. No one goes through it alone.

6

After

Our care for your family continues

Grief support runs for 13 months, and longer when it is needed. Grief counseling and support groups

Questions about any step?

Ask them before you decide anything. A nurse can walk you through the whole sequence in about ten minutes.

Start with a question, not a decision.

Nothing is set in motion by a phone call. A nurse answers, day or night.
